악관절 내장증환자에서 관절돌기의 후방경사에 관한 연구

Abstract

연구대상 총 78명 중 정상군 31명 62관절과 비정상군 47명 (편측성 35명, 양측성 12명)의
비정상관절 59관절을 대상으로 관절돌기의 후방경사도 및 관절돌기 후방경사와 과두간의 각
도를 연구 분석하여 다음과 같은 결과를 얻었다.
1. 비정상군의 비정상관절(Group Ⅰ)과 정상군의 정상관절(Group Ⅱ)의 관절돌기 후방경
사도는 각각 34.90°, 38.43°로 정상관절이 비정상관절보다 큰 것으로 나타났으나(p<0.05)
후방경사와 과두간의 각도는 각각 90.74°, 93.54°로 유의한 차이가 없었다.
2. 좌우측 양 관절간 계측 각도의 차이를 비교하면 편측성 환자(Group Ⅰ_a)
와 양측성 환자(Group Ⅰ_b), 정상군(Group Ⅱ)에서 관절돌기 후방경사도의 차
이는 각각 4.73°,6.46°, 1.24°이고 후방경사와 과두간의 각도의 차이는 각각 12.03°, 11,5
4°, 1.44°로 편측성 환자와 양측성 환자 모두에서 정상군보다 컸다(p<0.01)
3. 편측성 환자의 양관절을 비교하면 정상관절(Group Ⅰ_a1,)과 비정상관절
(Group Ⅱ_a2)에서 관절돌기 후방경사도는 각각 35.84°, 35.20° 후방경사와
과두간의 각도는 각자 93.26°, 91.71°로 정상관절의 각도가 비정상관절의 각도보다 모두
컸으나 유의한 차이는 없었다.
4. 비정상관절에서 관절원판 전방변위의 정복성 24예, 비정복성 26예, 섬유성 관절유착 9
예에서 관절돌기의 후방경사도를 비교한 결과 차이가 없었으나 관절돌기 후방경사와 과두간
의 각도를 비교한 결과는 각각 3경우가 서로 차이 (정복성 관절원판 전방변위<섬유성 관절
유착<비정복성 관절원판 전방변위)를 보였다(p<0.05).

This study was designed to investigate the effects of the posterior slope of the
articular eminence and the condylar angulation to the posterior slope of the articular
eminence on internal derangement of TMJ. The materials consisted of 78 transcranial
oblique lateral projections of 31 normal subjects and 47 internal derangement patients.
The results were as follows :
1. The posterior slope of the articular eminences in normal group were larger than
that in abnormal group, but there were not significant differences between each group
about the condylar angulation to the posterior slope of the articular eminences.
2. The differences between in the right and left sides in patients who were affected
unilaterally and bilaterally were larger than that in normal groups.
3. In patients affected unilaterally, there were not significant differences between
affected joints and unaffected joints about the posterior slope of the articular eminence
and the condylar angulation to the posterior slope of the articular eminence.
4. In abnormal group, there were significant differences between each group about
condylar angulation to the post, slope of the articular eminence (ant. disk displacement
without reduction > fibrous adhesion > ant. disk displacement with reduction), but not
about the post, slope of the articular eminence.
